Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Duties & Responsibilities Analyze patient's medical record for proper utilization or resources(beds and services) and accurate payment. Develop the Hospital data base for case management and severity implications. Analyze patient's charts for quality of services rendered and intervene to assure the delivery of quality care. Communicate to patient and/or patient's family as necessary regarding the medical necessity of hospital stay. Communicate to appropriate departments to accurately determine level of care at discharge as well as participate in discharge planning. Performs all other related duties as assigned. Requirements Bachelor's Degree in Nursing and a minimum of five year's job-related experience or equivalent is required. An RN license is also required.
