_PARAGRAPH JOB_DESCRIPTION.SHARE.HTML VNA of Care New England Warwick, Rhode Island 8230 Medical Nursing Part-Time Days Union Mon.-Fri. 24.00 Job Description Job Summary The RN Triage Home Health assists the clinical management team in providing day-to-day clinical triage coverage and project development. They serve as a liaison with clinical staff, physicians and patients by assisting in the triage of clinical concerns, providing guidance to staff and patients, assisting with physician orders and speaking with primary care physicians and referral sources. They also assist the Clinical Manager in the coordination of assignments as necessary and serve as a resource for team members and other departments on clinical and departmental issues. Duties & Responsibilities Assists Manager with the day-to-day clinical operations and project development, triaging clinical calls as necessary and in accordance with organization policies/procedures and standards consistent with Federal, State and Joint Commission regulations and/or delegating care to a Care Partner and/or other appropriate team member. Seeks and provides peer consultation about cases that are presenting problems and/or experiencing significant deviation from the Plan of Care for hospice and home health. Consistently reviews complex patient/family situations for hospice and home health with the Manager and, when appropriate, the Social Worker and other team members. Maintains current knowledge of organizational and departmental policies, procedures and protocols to act as a resource to hospice and home health team members and other departments. In conjunction with the Manager, coordinates orientation and education programs in accordance with the established organizational standards, guidelines, and in response to the organization educational needs assessment and plan for hospice and home health. Maintains current information regarding agency's programs in hospice and home health, along with information regarding community resources. Fosters working relationships and communication with employees, referral sources and patients. Provides back-up support during Manager's absences which may include flex hours to cover on call triage from 6-8 pm. Requirements Graduate of an accredited nursing program required. Bachelor's degree strongly preferred. Current Rhode Island Registered Nurse licensure. Must be CPR certified. Valid driver's license and current automobile liability insurance. Minimum total of two (2) to three (3) years nursing experience in a home care environment. Minimum of two (2) years clinical nursing experience in a medical/surgical or acute care environment. Previous experience within specific clinical area, based on assignment, preferred. Supervisory experience preferred. Shows initiative and good judgment. Excellent communications and organizational skills required. Must be able to work independently and complete assignments with minimal supervision. Must be able to coordinate prioritize and complete tasks simultaneously in a timely manner. Must be customer-service oriented and able to blend strong clinical knowledge with excellent problem-solving and organizational skills. Excellent interpersonal skills required. Must be able to perform duties effectively in stressful emergency situations while maintaining a professional demeanor. Demonstrates excellent time management skills to improve and maximize job functions.
