Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients prior and current medical histories, conditions, and medications Educate patients about their health conditions and medication regimens Detect care gaps and recommend solutions to optimize patient outcomes No treatment plan creation or medication prescribing required

Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification through AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing school At least 6 months of current NP experience preferred This role offers you the flexibility to choose your schedule and supports a healthy work-life balance.
