MDS Coordinator/RN - Long Term Care

Job Description

Job Summary We are seeking a dedicated and detail-oriented MDS Coordinator/RN to join our long-term care team. In this vital role, you will lead the comprehensive assessment and documentation process for residents, ensuring compliance with federal and state regulations, including those set forth by the Centers for Medicare and Medicaid Services (CMS). Your expertise will drive quality patient care, optimize clinical documentation, and support accurate reimbursement through meticulous coding and utilization management. Responsibilities Conduct thorough Minimum Data Set (MDS) assessments in accordance with clinical standards, regulatory guidelines, and facility policies to evaluate residents' functional capabilities and care needs. Review medical records, clinical documentation, and coding details such as
ICD-10/ICD-9
codes, CPT codes, DRGs, and HEDIS measures to ensure accuracy and completeness. Collaborate with interdisciplinary teams—including nursing, therapy, social services, and physicians—to develop individualized care plans aligned with residents' assessments. Manage electronic health records (EHR/EHR systems like Cerner, Epic, Athenahealth, or eClinicalWorks), ensuring timely updates and compliance with documentation standards. Oversee utilization review processes to optimize resource allocation while adhering to Medicare and Medicaid regulations; participate in discharge planning and case management activities. Maintain knowledge of long-term care regulations, CMS policies, HIPAA compliance, and state healthcare laws to ensure ongoing regulatory adherence. Provide education and guidance on clinical documentation improvement initiatives to enhance data quality for quality reporting programs such as NCQA standards. Qualifications Valid Registered Nurse (RN) license with current state licensure. Proven experience in long-term care settings or nursing homes; ICU or acute care experience is highly valued. Strong knowledge of medical terminology, physiology, anatomy, and clinical documentation standards. Familiarity with EMR/EHR systems such as Cerner, Epic, Athenahealth, or eClinicalWorks; proficiency in Microsoft Office is preferred. Experience with medical coding including
ICD-10/ICD-9
coding, CPT coding, DRGs, HEDIS measures, and utilization management processes. Understanding of Medicare regulations, CMS guidelines, HIPAA compliance, and long-term care regulations. Excellent case management skills with a focus on discharge planning and patient care coordination. Ability to interpret complex health information management data accurately while maintaining attention to detail.
