Schedule:

13-week contract, with extension potential Monday through Friday, full-time hours On-call responsibilities may be included Daily patient visit structure

Responsibilities:

Provide in-home patient assessments and skilled nursing care Complete thorough, accurate OASIS documentation in a timely manner Collaborate with physicians and multidisciplinary teams to implement individualized care plans Deliver wound care, medication management, patient/caregiver education, and monitoring for changes in health status Keep care teams informed with timely communication of patient updates

Desired Qualifications:

Active and unencumbered RN license At least one year of recent home health RN experience Demonstrated proficiency with OASIS documentation HomeCare HomeBase familiarity is highly preferred Excellent time management and organizational abilities Reliable transportation and a valid drivers license
