Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members Assess prior and current medical history, chronic conditions, and medication use Offer patient education on health conditions and prescription regimens Identify care gaps and recommend appropriate steps for improved outcomes No responsibility for developing treatment plans or prescribing medication

Qualifications:

Active Nurse Practitioner license for South Carolina Board Certification from AANP or

ANCC MSN
