Complete home health risk assessments for Medicare Advantage members in various locations across Dillon County Review and evaluate patients previous and current medical history, conditions, and medications Offer individualized education regarding medical conditions and treatment regimens Identify care gaps, offering practical recommendations to improve patient outcomes No prescribing or formulation of treatment plans required

Qualifications:
