Schedule:

Full-time, Monday through Friday On-call duties may be required Structured daily patient visits 13-week contract with potential for extension

Qualifications:

Active RN license in good standing At least 1 year of recent RN home health experience required Demonstrated proficiency with OASIS documentation Familiarity with HomeCare HomeBase (strongly preferred) Reliable transportation and valid driver's license Comfortable managing a set, full-time schedule with multiple daily visits

Key Responsibilities:

Deliver in-home nursing care, including patient assessments and skilled interventions Complete OASIS documentation accurately and promptly Create and implement individualized care plans in collaboration with physicians and interdisciplinary teams Perform wound care, medication administration, patient teaching, and disease management Communicate effectively with clinical teams regarding changes in patient condition
