Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members in their residences Review and document patients prior and current medical history, conditions, and medication use Provide educational guidance on health conditions and prescribed medications Identify gaps in care and recommend solutions to optimize patient outcomes No responsibility for developing treatment plans or prescribing medications

Qualifications:
