Key Responsibilities:

Conduct annual home health risk assessments during 45-minute in-home visits Evaluate and document prior/current medical history, chronic/acute medical conditions, and medication usage Provide patient education concerning medical conditions and prescribed medications Assess gaps in care, and offer practical recommendations to enhance overall patient health Complete only health assessmentsno requirement to develop treatment plans or prescribe medication

Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification from AANP or ANCC Current CPR certification (BLS) Master's degree in Nursing (MSN) from an accredited program Minimum of six months experience as an NP preferred
