Key Responsibilities:

Conduct comprehensive annual risk assessments in members homes across Dillon, Latta, Lake View, and surrounding areas Review patients prior and current medical history, assess chronic conditions, and update medication profiles Educate individuals on disease management, prevention, and proper medication usage Detect gaps in care and provide actionable recommendations to close them Focus on assessment and education onlyno medication prescribing or treatment planning is required Time per assessment averages 45 minutes

Qualifications:

Unrestricted Nurse Practitioner license in South Carolina Board certification from AANP or ANCC Current CPR certification (BLS required) MSN degree from an accredited school of nursing At least 6 months of current NP experience preferred This role offers competitive compensation: $80-$90 per evaluation, along with a mileage stipend and paid administrative time.
