Key Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage members Review and evaluate a patients medical history, current conditions, and medication regimen during approximately 45-minute home visits Educate patients about their medical conditions and prescribed medications Identify care gaps and recommend solutions to optimize health outcomes No requirement to prescribe medications or develop ongoing treatment plans

Qualifications:

Valid Nurse Practitioner License in South Carolina Current BLS (CPR) certification Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of experience working as an NP preferred
