Responsibilities:

Complete thorough home health risk assessments, typically lasting around 45 minutes per visit Review patient medical history, current health status, and medication use Identify care gaps and deliver clear recommendations to address them Educate patients regarding their medical conditions, treatment options, and medications Visits may include annual health assessments or post-discharge evaluations No requirement for establishing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification through AANP or ANCC Current CPR (BLS) certification Master of Science in Nursing (MSN) from an accredited university Minimum of 6 months experience as a Nurse Practitioner is strongly preferred Confident in performing independent patient assessments in a home-based setting Along with flexibility and autonomy, this role offers compensation of $80-$90 per evaluation, in addition to a mileage stipend and pay for administrative time.
