Overview We are seeking a highly motivated and detail-oriented MDS Coordinator to join our healthcare team. Duties Lead the development, completion, and accuracy of the Minimum Data Set (MDS) assessments in accordance with CMS regulations and clinical standards. Review medical records, clinical documentation, and coding (ICD-10, CPT) to ensure completeness and compliance with healthcare policies. Collaborate with nursing staff, physicians, case managers, and other healthcare professionals to gather pertinent patient information for accurate assessments. Conduct thorough documentation reviews to support quality measures such as HEDIS and NCQA standards, ensuring data integrity for regulatory reporting. Manage electronic health records (EHR/EHR systems like Cerner, Epic, Athenahealth) to maintain up-to-date patient information and facilitate seamless communication across care teams. Perform utilization management by reviewing hospitalizations, discharge planning, hospice care, and outpatient services to optimize patient outcomes while adhering to Medicare and long-term care regulations. HIPAA, CMS

guidelines, state healthcare regulations, and long-term care standards to ensure ongoing compliance. Qualifications RN a must Proven experience with MDS coordination in long-term care or skilled nursing facilities; ICU or acute care experience is highly desirable. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, DRG systems, and clinical documentation standards. Familiarity with EMR/EHR systems such as Cerner, Epic, eClinicalWorks or Athenahealth; proficiency in Microsoft Office Suite is preferred. Understanding of Medicare regulations, HEDIS measures, NCQA standards, and CMS requirements related to long-term care and hospital settings. Excellent organizational skills with the ability to review complex medical records efficiently while maintaining attention to detail. Ability to work collaboratively within interdisciplinary teams in a fast-paced environment while managing multiple priorities effectively. Knowledge of health information management practices related to discharge planning, case management, utilization review, and medical management processes.
