OASIS What Youll Do:

Complete comprehensive OASIS assessments, developing individualized care plans for each patient Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management Monitor patient conditions, evaluating progress and communicating timely updates to the care team and physicians Educate patients and caregivers on health conditions, treatment plans, medication regimens, and preventive strategies Maintain accurate, up-to-date clinical records in HCHB Collaborate with other healthcare professionals, including therapists, social workers, and aides, to ensure coordinated care Attend regular case conferences, staff meetings, and pursue ongoing education Comply with regulatory guidelines and standards for home health care delivery

Qualifications:

Active RN license in good standing for the state of practice Minimum 1 year of recent home health field experience required Proficient in Home Care Home Base (HCHB) and familiar with OASIS assessments Reliable transportation, valid drivers license, and current auto insurance
