Responsibilities include:
Completing comprehensive patient assessments Developing and updating individualized care plans Providing skilled nursing care and patient education Coordinating care with physicians and the interdisciplinary team Monitoring patient progress and documenting visits accurately Educating patients and caregivers to promote independence and prevent hospital readmissions Qualifications Current Tennessee Registered Nurse (RN) license Home Health experience preferred Current driver's license and reliable transportation CPR certification (or willingness to obtain) Strong assessment, communication, and clinical decision-making skills Commitment to compassionate, patient-centered care Why Volunteer Home Care?
