Complete comprehensive home health risk assessments for Medicare Advantage members Review patients medical history, current conditions, and medication regimen Offer education regarding medical conditions and medications during each assessment Identify care gaps and recommend solutions for improved health outcomes No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Tennessee Current Basic Life Support (BLS/CPR) certification Board certification via AANP or

ANCC MSN
