Primary Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members Review patients prior and current medical histories, conditions, and medication usage Educate patients on their medical conditions and medications for better self-management Identify any gaps in care, providing actionable recommendations to improve patient outcomes Documentation is required, but no treatment plans or medication prescribing needed

What Were Seeking:

Valid Nurse Practitioner license in Tennessee Board Certification through either AANP or ANCC Current CPR (BLS) certification MSN degree from a fully accredited nursing school

Preferred:

6 months or more of direct nurse practitioner experience, especially in home health

Compensation:

Earn between $80 and $90 per evaluation, supplemented by a mileage stipend and paid administrative time for each visit.
