Key Responsibilities:

Perform thorough annual home health risk assessments for Medicare Advantage members Review and evaluate patient medical, surgical, and medication histories Conduct assessments that may include annual wellness checks or post-discharge visits Educate patients about their conditions, recommended medications, and preventive actions Identify gaps in care and collaborate on recommendations for better health outcomes No prescriptive authority or treatment plan creation required

Qualifications:
