Key Responsibilities:

Perform thorough annual home health risk assessments for Medicare Advantage members Review and evaluate patient medical, surgical, and medication histories Conduct assessments that may include annual wellness checks or post-discharge visits Educate patients about their conditions, recommended medications, and preventive actions Identify gaps in care and collaborate on recommendations for better health outcomes No prescriptive authority or treatment plan creation required

Qualifications:

Active Nurse Practitioner license in Tennessee Up-to-date BLS (CPR) certification Board certification through AANP or ANCC At least six months of recent Nurse Practitioner experience preferred MSN degree from an accredited nursing institution This position offers compensation of $80-$90 per completed evaluation, plus a mileage stipend and additional pay for administrative time.
