Seeking an experienced RN to deliver exceptional patient care in a home health setting. This 13-week, full-time contract offers the opportunity to provide vital services in a field-based role, supporting patients within their homes. The position features a consistent Monday through Friday schedule, 8:00 AM 5:00 PM, with a manageable caseload of 30 points per weekusually 3 to 5 patient visits daily.

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop personalized care plans. Perform skilled nursing care, including medication administration, wound care, IV therapy, and management of chronic conditions. Monitor patient progress, reporting significant changes to the interdisciplinary team and physicians. Educate patients and families regarding diagnoses, treatment regimens, preventative measures, and medications. Maintain precise and up-to-date documentation in the Home Care Home Base (HCHB) system. Collaborate effectively with physical therapists, occupational therapists, social workers, and home health aides. Take part in case conferences, staff meetings, and ongoing educational sessions to uphold best practices. Ensure compliance with regulatory standards and agency care protocols. Travel daily within the assigned area, receiving mileage reimbursement for visits.

Qualifications:

Current, active RN license in good standing in the state of practice. At least one year of recent field-based home health experience. Proficiency in Home Care Home Base (HCHB) and familiarity with OASIS documentation. Reliable transportation, valid driver's license, and maintained auto insurance.
