Home Health Responsibilities:
Provide skilled nursing visits in the home setting according to physician, nurse practitioner, or physician assistant orders Perform patient assessments and develop nursing care plans Educate patients and families on medications, disease management, and recovery goals Monitor patient progress and report changes in condition to providers Document all patient care using the electronic medical record (EMR) Collaborate with therapists, aides, and case managers to support patient outcomes Participate in team meetings and patient care conferences Participate in the on-call rotation Registered Nurse (RN)Home Health Qualifications:
Active Registered Nurse (RN) license- Tennessee Current CPR certification Valid driver's license Reliable transportation and current auto insurance Home Health RN experience preferred Work Setting Home Health Field-based nursing Patient homes in the Huntingdon, TN service area RN Job Type Full-time Day shift Monday to Friday ________________________________________ This position may also be known as: Home Health RN, Registered Nurse, RN Case Manager, Field Nurse, Visiting Nurse, Community Health Nurse, Home Care RN, RN Care Coordinator.
