to deliver high-acuity bedside care in long-term acute care settings. Responsibilities Provide direct high-acuity nursing care to LTAC patients, including complex medical and rehab needs Perform comprehensive assessments, monitor vitals, and respond promptly to changes in patient condition Administer medications, manage IVs, ventilators, and tracheostomies per facility protocols Coordinate with physicians, therapists, and care teams to execute and update individualized care plans Document care accurately in the EMR, ensuring compliance with facility and regulatory standards Educate patients and families on treatment plans, recovery expectations, and discharge needs Adhere to infection control, safety policies, and quality standards to support optimal outcomes Float as needed within LTAC settings, demonstrating flexibility and strong clinical judgment Required Skills LTAC nursing care High-acuity patient assessment Ventilator and trach management Wound care and dressing changes IV therapy and medication administration Care planning and documentation (EMR) Infection prevention and control Interdisciplinary team collaboration Patient and family education Time management and prioritization
