Gifted Healthcare is seeking a PRN Telehealth RN in Jackson, TN. In this role, you'll deliver remote patient care via phone and video, performing triage, assessments, and ongoing monitoring. You will collaborate with providers to implement care plans, document accurately in the EHR, and educate patients and families on conditions and self-care. Responsibilities Provide direct telehealth RN care to patients, including assessment, triage, and care coordination via phone or video platforms Monitor patient conditions, respond to changes, and escalate concerns to providers as appropriate Document all patient interactions, assessments, and interventions accurately in the electronic health record (EHR) Collaborate with physicians, advanced practice providers, and interdisciplinary teams to support care plans Educate patients and families on conditions, medications, and self-care using clear, empathetic communication Follow all clinical protocols, safety standards, and regulatory requirements for remote nursing care Utilize telehealth technology and remote monitoring tools effectively and troubleshoot basic technical issues Participate in scheduling flexibility to cover PRN shifts, evenings, nights, weekends, and holidays as needed Protect patient privacy and maintain HIPAA compliance in all communications and documentation Contribute to a culture of respect, integrity, and clinical excellence aligned with Gifted Healthcare's mission Required Skills Telephone triage Telehealth platforms and workflows Remote patient monitoring Clinical assessment and critical thinking Care coordination and case management Electronic Health Record (EHR) documentation Patient and family education HIPAA and regulatory compliance Acute and chronic disease management Time management and multitasking
