Schedule:

Monday Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (approximately 35 patient visits daily)

Key Responsibilities:

Perform comprehensive OASIS assessments and develop customized care plans for home health patients. Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease support. Closely monitor patient status, document in Home Care Home Base (HCHB), and communicate promptly with the care team and physicians. Provide education to patients and their families on medical conditions, medications, and preventive practices. Ensure accurate, timely electronic documentation in compliance with regulatory and agency guidelines. Collaborate with interdisciplinary team members, participating in case conferences and staff meetings for coordinated care. Attend continuing education and adhere to all standards for high-quality patient care.

Qualifications:
