As a nurse in our facility, you will be responsible for delivering comprehensive healthcare services, coordinating patient assessments, and ensuring adherence to safety and regulatory standards. This role offers an opportunity to make a meaningful difference in the lives of seniors and individuals with diverse healthcare needs. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health records (EHR) systems such as Cerner or Athenahealth. Administer medications and injections accurately, including IV infusions, botulinum toxin treatments, and other specialized therapies. Monitor patients' conditions closely, managing acute pain, respiratory support (ventilators), and infection control protocols in compliance with HIPAA regulations. Provide wound care, including suturing and ostomy management, while maintaining aseptic techniques to prevent infections. Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care strategies tailored to residents' needs. Assist with diagnostic evaluations such as laboratory specimen collection, radiology procedures like CT scans or sonography, and cardiac catheterization preparations. Support residents with memory care, dementia, Alzheimer's disease, and behavioral health needs through personalized care plans. Ensure compliance with healthcare standards including NCQA standards for quality assurance and ICD coding for documentation accuracy. Skills Proficiency in EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical documentation and records management. Extensive experience in various nursing settings including hospital medicine, long-term care facilities, assisted living communities, and home care environments. Skilled in critical care procedures such as ICU or Level I Trauma Center experience; familiarity with trauma medicine and acute care protocols. Expertise in specialized treatments including dialysis management, tube feeding administration, ostomy care, and neonatal resuscitation procedures. Strong knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to geriatrics and pediatric populations. Ability to perform complex procedures such as catheterization (urinary or vascular), phlebotomy (venipuncture), and sterile processing techniques. Excellent communication skills for patient education on health coaching topics like medication administration or lifestyle modifications; adept at triage assessments in urgent care or emergency room settings. Familiarity with Workers' Compensation law related to occupational health cases; experience working with individuals with disabilities or developmental delays is preferred.
