Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members Review and evaluate each patients medical history, current conditions, and medication profiles Educate patients regarding their medical conditions and medications to enhance understanding and compliance Identify care gaps and offer recommendations for improved patient outcomes No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Tennessee Board Certification as a Nurse Practitioner through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of current NP experience preferred As part of this rewarding per diem opportunity, you will receive competitive compensation of $80-$90 per evaluation, along with a mileage stipend and coverage for administrative time.
