PRN LTAC RN

to deliver high-quality, patient-centered care in a Long-Term Acute Care setting. In this flexible PRN role, you will assess, plan, and implement complex care for medically fragile, high-acuity patients, including ventilator and trach management. You'll collaborate with an interdisciplinary team, ensure accurate documentation, support infection control and patient safety standards, and educate patients and families. Responsibilities Provide direct nursing care to LTAC patients, including assessment, planning, implementation, and evaluation of care plans Administer medications, treatments, and interventions per physician orders and facility protocols Monitor and manage complex, high-acuity patients with ventilators, trachs, and multiple comorbidities Collaborate with physicians, therapists, and interdisciplinary team members to optimize patient outcomes Document nursing interventions, patient responses, and changes in condition accurately and timely in the EMREducate patients and families on treatment plans, disease processes, and discharge needs Adhere to infection control, patient safety, and regulatory standards in an LTAC environment Respond to emergencies, perform rapid assessments, and initiate appropriate interventions Support a culture of respect, integrity, and evidence-based practice Float and adapt to varying LTAC units and shifts as PRN needs arise Required Skills Ventilator management Tracheostomy care IV therapy and medication administration Wound care and pressure injury prevention Cardiac and respiratory assessment Telemetry monitoring Infection prevention and control Electronic Medical Records (EMR) documentation Care planning and care coordination Emergency response and rapid assessment
