Responsibilities include:

Completing skilled nursing visits according to physician, nurse practitioner, or physician assistant orders Performing patient assessments and developing individualized care plans Educating patients and families on medications, disease management, and recovery goals Monitoring patient progress and communicating changes in condition to providers Documenting care accurately using an electronic medical record (EMR) Collaborating with therapists, aides, and care team members to achieve positive patient outcomes Participating in team meetings, patient care conferences, and quality improvement activities Participating in an on-call rotation

Qualifications Required:

Current Tennessee Registered Nurse (RN) license Current CPR certification Valid driver's license Reliable transportation and current auto insurance

Preferred:

Previous Home Health RN experience Experience with OASIS documentation and Medicare home health requirements
