Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members in designated areas Assess medical history, current conditions, and medication usage Educate patients on health conditions and medications Identify gaps in care and recommend solutions No requirement to create treatment plans or prescribe medications

Qualifications:

Current Nurse Practitioner License in Tennessee CPR (BLS) certification required Board Certification through AANP or ANCC At least 6 months of recent Nurse Practitioner experience is preferred Master of Science in Nursing (MSN) from an accredited institution This per diem role offers a pay rate of $80$90 per evaluation and includes additional stipends for mileage and administrative time.
