Duties Conduct detailed patient assessments focusing on chronic disease management, including vital signs, physiology evaluation, and health history review. Manage patient cases through clinical case management techniques, utilizing electronic health records (EHR) systems for documentation and communication. Provide education and support to patients and their families on managing chronic conditions, medication adherence, lifestyle modifications, and discharge planning. Coordinate with primary care providers, specialists, home health agencies, hospice care teams, and other healthcare entities to ensure seamless continuity of care. Monitor utilization management and conduct diagnostic evaluations to optimize treatment strategies while adhering to health regulation policies such as HIPAA. Engage in patient service activities including intake procedures, post-acute care follow-up, outpatient visits, and inpatient support within hospital medicine or emergency room settings. Qualifications Proven experience in critical care environments such as ICU or Level I/II trauma centers; experience in hospital medicine or emergency medicine is highly desirable. Strong background in infectious disease care, geriatrics, behavioral health, or pediatric intensive care (PICU), with familiarity in managing complex cases across inpatient and outpatient settings. Knowledge of medical terminology, physiology principles, medical records management, and electronic health record (EHR) systems like EMR/EHR platforms. Demonstrated ability to perform comprehensive patient assessments including vital signs and clinical evaluations; experience managing patients as a nurse case manager is preferred. Familiarity with healthcare regulations such as HIPAA and experience in discharge planning and post-acute care coordination. Excellent client communication skills with the ability to collaborate effectively with multidisciplinary teams of healthcare professionals. Certification in nursing (RN license) with additional training or experience in managed care or utilization management is advantageous. Ability to adapt quickly within fast-paced hospital or clinic environments such as emergency rooms or outpatient clinics; prior home health or hospice experience is a plus.
