This per diem position centers on serving Medicare Advantage plan members in Brazos County and its surrounding areas, including College Station, Bryan, Navasota, and Wixon Valley. Key Responsibilities:

Perform comprehensive annual home health risk assessments in patient homes Review and document previous and current medical history, health conditions, and medication use Provide patient education on chronic conditions, disease management, and medication regimens Identify potential gaps in care and recommend actionable next steps No ongoing care plans or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Texas Current BLS (CPR) certification Board Certification as an NP through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution At least 6 months of current NP experience preferred
