BASTROP LOST PINES NURSING AND REHABILITATION CENTER - BASTROP, TX
Are you a licensed nurse with a passion for wound care and skin health? Wellsential Health is seeking a dedicated Treatment Nurse (LVN or RN) to help prevent, monitor, and manage skin integrity issues for residents in our skilled nursing and long-term care facility. This is a crucial role in ensuring each resident receives quality, person-centered care that supports healing and comfort.
Key Responsibilities:
Deliver high-quality wound care treatments in accordance with physician orders and best practices Perform and document weekly skin and pressure ulcer assessments Communicate changes in skin integrity to the Director of Nursing (DON) and other clinical team members Accurately apply and change dressings per treatment orders and on schedule Track healing progress and recommend changes to treatment plans when necessary Educate residents, families, and staff on wound prevention, skincare, and infection control Document all findings on the Weekly Focused Skin Assessment and share results during QA and care plan meetings Collaborate with physicians and responsible parties regarding resident skin conditions Participate in in-service training and stay current on wound care practices
Qualifications:
Graduate of an accredited nursing program (LVN or RN) Active RN or LVN license in the state of Texas Wound Care Certification (WCC) preferred but not required Previous experience in skilled nursing, long-term care, or wound care strongly preferred Detail-oriented, compassionate, and committed to improving residents' quality of life Strong documentation and communication skills When you join our team at Wellsential Health, a non-profit company, you become part of a mission-driven organization that provides each person in our community with the support they need to pursue better health, engage in their world, and fuel their purpose.
