Key Responsibilities:

Complete comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members Review, document, and evaluate prior/current medical histories, existing conditions, and medication usage Educate patients about their medical conditions and prescribed medications Highlight and address any care gaps, offering clear recommendations to support improved health outcomes Focus on assessment and education; no diagnosis, creation of treatment plans, or prescribing required

Desired Qualifications:

Active Nurse Practitioner license in Texas Board Certified through AANP or ANCC Current BLS (CPR) certification MSN degree from an accredited nursing program A minimum of six months NP experience in a related clinical setting is preferred
