Home health risk assessments typically last about 45 minutes and involve comprehensive annual visits, post-discharge assessments, and focused evaluations with Medicare Advantage members.

Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members Review and document medical histories, current conditions, and medication regimens Provide patient education on disease management and medication adherence Identify gaps in care and recommend actionable next steps to support continuity of care No treatment plan development or medication prescribing required

Qualifications:

Active Nurse Practitioner (NP) license in Texas Current BLS/CPR certification Board Certification from AANP or ANCC Masters degree in Nursing (MSN) from an accredited program At least 6 months of recent NP experience preferred In return for your expertise, you will receive competitive compensation of $80-$90 per completed evaluation, along with additional mileage reimbursement and compensation for administrative tasks.
