A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Job Description

Company Overview:
Solaris is clinician owned and operated and has been recognized as industry experts in palliative medicine, since 1998. We are committed to helping patients and their families always put life first.
Responsibilities:
  • Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
  • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
Qualifications and Requirements:
  • Graduate of an accredited school of nursing.
  • Current RN licensing, in good standing.
  • Valid drivers license, and auto insurance
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
