Job Description

Methodist Healthcare in San Antonio seeks a Transplant ICU Clinical Nurse Coordinator to lead and coordinate complex care for transplant patients in a high-acuity ICU. In this mission-driven, collaborative environment, you will manage patient flow, develop care plans, and serve as a key liaison between physicians, nurses, patients, and families. Responsibilities include patient education, regulatory compliance, quality improvement, and supporting staff development while ensuring safe, compassionate, evidence-based care for transplant recipients. Responsibilities Coordinate and manage care for transplant ICU patients from admission through discharge Serve as primary clinical resource and liaison among physicians, nurses, patients, and families Develop, implement, and evaluate individualized care plans for complex transplant cases Monitor patient status, interpret clinical data, and escalate changes in condition promptly Ensure compliance with transplant protocols, regulatory standards, and quality/safety initiatives Educate patients and families on transplant procedures, recovery, and post-discharge care Participate in multidisciplinary rounds and collaborate on care planning and transitions Support staff education, precept new team members, and promote evidence-based practice Document assessments, interventions, and outcomes accurately in the EMRContribute to continuous improvement projects and transplant program outcomes tracking Required Skills Transplant ICU nursing Critical care assessment Care coordination Patient and family education Clinical protocol management Hemodynamic monitoring Ventilator management Electronic medical records (EMR) documentation Interdisciplinary team collaboration Quality and regulatory compliance
