Primary Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review medical histories, current conditions, and prescribed medications Deliver education to patients about their health status and medication management Identify and document any care gaps, providing recommendations for follow-up No requirement to create treatment plans or prescribe medications

Desired Qualifications:
