New Grads Accepted - Must have an active RN license Admission and Case Management of patients eligible for hospice services. Completes initial assessment, comprehensive assessment and updated assessments that specifically addresses the identified patient problems, nursing problems and goals. Provides intermittent skilled nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the Patient Plan of Care. Submits documentation (nursing notes, communication notes, hospice aide supervisory notes) per agency policy. Participates in IDG meetings every 2 weeks. Effectively communicates with all members of IDG team. Acts as patient advocate. Makes on site visit to patient's home no less than every 14 days. Attends staff meetings and in-services per agency requirements. Reports to
