Schedule:

Flexible shifts available Monday through Sunday Self-scheduling; set your preferred hours Commit to a minimum weekly or bi-weekly hourly availability

Key Responsibilities:

Complete annual health risk assessments in members homes, each visit averaging about 45 minutes Review prior and current medical histories, conditions, and medications Educate patients about their health conditions and medications Assess and identify gaps in care, providing recommendations to improve health outcomes No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Texas Current BLS certification (CPR) Board certification through AANP or ANCC Minimum of 6 months of recent nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program Enjoy competitive pay for each completed evaluation, along with an additional mileage stipend and compensation for administrative time.
