We are seeking a Full-Time Home Health RN Case Manager who is confident in clinical assessment, care coordination, OASIS documentation, and developing individualized plans of care. Why Join Our Team?

  • Full-time schedule with occasional weekend visits Position Responsibilities As a Home Health RN Case Manager, you will:
  • Provide skilled nursing care to patients in their homes
  • Complete comprehensive assessments and ongoing clinical evaluations
  • Develop, implement, evaluate, and update individualized Plans of Care
  • Complete accurate and timely OASIS assessments and required documentation
  • Identify changes in patient condition and implement appropriate interventions
  • Collaborate with physicians, patients, families, caregivers, and other members of the healthcare team
  • Provide patient and caregiver education related to disease management, medications, safety, and self-care
  • Monitor patient progress toward goals and revise the plan of care as clinically indicated
  • Participate in recertification, resumption of care, follow-up, and discharge planning
  • Help prevent avoidable hospitalizations through proactive assessment, education, and coordination of care
  • Maintain compliance with Medicare, agency, and applicable regulatory requirements We are looking for an RN who is:
  • Clinically confident and highly organized
  • Detail-oriented with strong OASIS and documentation skills
  • Able to work independently while functioning as part of a collaborative team
  • Skilled in patient assessment, care planning, and clinical decision-making
  • Compassionate, dependable, and committed to providing high-quality patient care
  • Comfortable coordinating care with patients, families, physicians, and interdisciplinary team members Qualifications
  • Graduate of an accredited school of professional nursing
  • Current, unrestricted RN license in the state of Texas
  • Current driver's license and reliable transportation
