• Responsibilities include:
• + Performing comprehensive nursing assessments in patients' homes.

+ Developing, implementing, and updating individualized plans of care. + Providing skilled nursing care, treatments, and patient education. + Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care. + Monitoring patient progress and communicating changes in condition. + Educating patients and caregivers to promote confidence and independence. + Maintaining timely and accurate clinical documentation. + Delivering care in accordance with regulatory and organizational standards.

• _Qualifications_
• Required
• + Current, unrestricted Texas Registered Nurse (RN) license + Minimum one (1) year of nursing experience + Strong assessment and critical thinking skills + Valid driver's license, reliable transportation, and current auto insurance + Ability to travel throughout the assigned service area

• Physical Requirements:

  Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support 50-100 pounds, including assisting with patient transfers and moving equipment.
• Preferred
• + Home Health or Hospice experience + OASIS documentation experience + Homecare Homebase (HCHB) experience + Associate or Bachelor's Degree in Nursing
