Full-Time Salary Range:
$85K-$95K (Mileage reimbursement .65 per mile, $60.00 per month cell phone reimbursement) Position Overview We are seeking a compassionate and experienced Hospice Admissions Registered Nurse (RN) to serve as the first clinical point of contact for patients and families beginning their hospice journey. This role combines strong clinical assessment skills with exceptional communication to ensure a seamless and supportive admission experience.
Responsibilities:
  • Evaluate and assess patients to determine hospice eligibility.
  • Guide patients and families through the hospice admission process with compassion and professionalism.
  • Develop individualized plans of care for newly admitted patients.
  • Collaborate with the interdisciplinary care team to ensure smooth patient transitions.
  • Maintain accurate and timely clinical documentation.
  • Participate in limited on-call responsibilities as required.
  • Support additional clinical and administrative duties as assigned.
Qualifications:
  • Provide skilled nursing care based on individualized care plans.
  • Current, active Texas Registered Nurse (RN) License.
  • Minimum 1 year of Hospice or Senior Care experience required.
