In this role, you will provide high-quality nursing care to patients in their homes or hospice facilities, ensuring comfort and dignity during end-of-life care. You will work closely with patients, families, and interdisciplinary teams to deliver comprehensive care tailored to individual needs. Duties Conduct thorough assessments of patients' health status, including vital signs and diagnostic evaluations. Develop and implement individualized care plans in collaboration with the healthcare team. Administer medications and treatments as prescribed, including catheter care and ventilatory support when necessary. Monitor patients for changes in condition and respond appropriately to emergencies. Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life decisions. Maintain accurate medical documentation of patient care activities and progress. Assist with case management tasks, coordinating services and resources for patients as needed. Lift and transfer patients safely while adhering to best practices for patient handling. Requirements Valid nursing license (RN or LPN) in the state of practice. Experience in nursing home or ICU settings preferred. Proficiency in assessing vital signs and performing diagnostic evaluations. Strong skills in case management and medical documentation. Ability to lift patients safely using appropriate techniques. Excellent communication skills with a focus on empathy and patient-centered care.
