Schedule:

13-week contract duration, with potential for extension Full-time, Monday through Friday On-call responsibilities may be required

Key Responsibilities:

Deliver in-home nursing care, including skilled interventions tailored to each patient's needs Conduct comprehensive assessments and complete all OASIS documentation promptly and accurately Collaborate with physicians and the care team to implement individualized care plans Perform wound care, medication administration, patient education, and monitor chronic conditions Communicate any changes in patient status or care needs effectively with the clinical team

Qualifications:

Current and active RN license At least 1 year of recent home health RN experience OASIS documentation expertise is required Strong proficiency with HomeCare HomeBase (preferred) Consistent ability to manage a structured schedule and daily visit expectations Reliable transportation and a valid drivers license are mandatory
