is seeking a compassionate and dependable Registered Nurse (RN) to provide nursing support to adults receiving services through our Home and Community-Based Services - Adult Mental Health (HCBS-AMH) program. This is a community-based mental-health nursing position for an RN who enjoys working one-on-one with adults and providing supportive, preventive, and medication-related nursing care in a home and community setting. What You'll Do Visit clients in their homes and community settings. Monitor vital signs and general health status. Monitor medications, medication adherence, and potential side effects. Communicate with clients regarding their health, medications, and well-being. Observe and report changes in physical or behavioral health. Communicate with physicians, behavioral-health professionals, case managers, and other members of the care team. Provide basic health education and reinforce treatment recommendations. Complete required nursing documentation in a timely manner. Help coordinate appropriate medical and behavioral-health care when concerns arise. This Is NOT Traditional Skilled Home Health This position does not routinely involve wound care, IV therapy, complex treatments, or high-acuity skilled nursing procedures . The focus is on psychiatric/community nursing, medication monitoring, basic health assessment, client communication, and coordination of care . What We're Looking For Current Texas RN license in good standing Psychiatric, behavioral-health, community-health, or mental-health experience preferred Strong communication and interpersonal skills Compassion, patience, and professionalism Ability to work independently in clients' homes and community settings Ability to maintain accurate and timely documentation Why This Position?
