Primary Responsibilities:

Perform annual risk assessments in patients homes Evaluate prior and current medical history, health conditions, and medications Educate patients on disease management and proper medication usage Identify gaps in care and provide actionable recommendations No requirement to create treatment plans or prescribe medications

Desired Skills and Qualifications:

Active Texas Nurse Practitioner license MSN degree from an accredited nursing program Board certification through AANP or

ANCC CPR

certification (BLS) Preferably at least 6 months of current NP experience, particularly in home health Strong interpersonal skills and ability to adapt to varied home settings You will earn $80-$90 per evaluation, plus receive a mileage stipend and additional compensation for administrative duties.
