Hospice LVN — Full-Time, Mon-Fri (Laredo)

Join a Patient-Centered Hospice Team in Laredo Redeemer Hospice is a Medicare-certified hospice agency serving patients across Webb County. We're hiring a dedicated full-time LVN to provide compassionate, high-quality care to patients in their homes during their most meaningful chapter of life. This role is ideal for an LVN who values continuity of care, manageable patient panels, and working with a stable interdisciplinary team — not running between facilities all day. What You'll Do Conduct routine home visits to a consistent panel of 12-15 hospice patients in the Laredo area Perform symptom assessments, vital signs, medication management, and wound care under RN supervision Document clinical observations accurately in our EMR system Educate patients and families on disease progression, comfort care, and medication use Coordinate care with the Interdisciplinary Team (RN, MSW, Chaplain, HHA, Medical Director) Communicate clinical changes promptly to the RN Case Manager Provide quality care that honors patient dignity and family preferences Schedule Monday-Friday, 8:00 AM - 5:00 PM Approximately 4-5 patient visits per day (manageable, achievable, defined panel) Compensation & Benefits $33.00 per hour ($68,640/year base at 40 hours/week) Mileage reimbursement Paid time off accrual Direct deposit, biweekly pay

What We're Looking For Required:

Active Texas LVN license in good standing (Texas Board of Nursing) Valid Texas driver's license, reliable vehicle, current auto insurance Ability to travel within Webb County and surrounding areas Bilingual (English/Spanish) — the majority of our patients speak

Spanish Preferred:

Minimum 1 year hospice or home health experience Familiarity with EMR documentation Wound care certification (WCC) or experience Why Redeemer Hospice Manageable patient panel — you'll see the same 12-15 patients consistently, not 25+ in chaos Strong RN supervision — our ADON works closely with field staff, not from a distant office Local, family-feel agency — decisions get made in San Antonio and Laredo, not a corporate office in another state Direct access to leadership — you talk to the Administrator, not a layer of middle managers Stable interdisciplinary team — including dedicated MSW, Chaplain, HHA, and Medical Director How to Apply Apply directly through Indeed with your resume and Texas LVN license number.
