Your Role:
Conduct physical exams, manage both acute and chronic conditions. Manage medication regimens and polypharmacy. Develop comprehensive care plans including collaboration with primary care physicians, home health or hospice services. Offer personalized patient and family education and support, with on-call availability for nursing facility staff.

APRN licensed in Texas, with effective communication skills and experience in geriatric, adult, acute, or family medicine.
