Key Responsibilities:

Complete annual home health risk assessments in patients homes for a Medicare Advantage health plan Assess prior and current medical history, documented medical conditions, and comprehensive medication use Educate patients on managing medical conditions and understanding medications Identify and communicate gaps in care while recommending steps for improvement No development of treatment plans or prescription responsibilities required

Qualifications:

Active, unencumbered Nurse Practitioner license in Texas Current CPR (BLS) certification Board Certification as an NP through AANP or ANCC Completion of an MSN degree from an accredited nursing program At least 6 months of clinical Nurse Practitioner experience preferred This per evaluation opportunity provides compensation of $80-$90 per assessment completed, with additional stipends included for mileage and administrative tasks.
